Started off intermittent 1/5 times car was started:
Noise/level sounds a lot like failing power steering pump.
    Clutch slippage at high rpm.

Noise level jumped 10 fold, prompting video below. Depressing clutch, noise goes away by degree of disengagement. Other than the noise, shifts fine, all gears working. Shut down car, next time started, no noise at all.

Fairly certain it's clutch/throw out bearing and due for replacement along with clutch. Wanted to hear from forum member's experience to confirm.

    Thanks!

I had a similar noise on startup. Was very intermittent initially, got worse over time. Throw out bearing in my case. Don’t forget to do the pilot bearing as well while you’re in there.

    Thanks AVIMAX.

Did you consider changing the torque tube bearings?

I did not. New clutch, new flywheel (long story), new throwout bearing and new pilot bearing. Clutch is beautiful now.
