    Sad, and in hindsight there were some indications of problems, which means maybe they were preventable, BUT protocol changes have been made. And, if it's only happened 2x in 33 years, those are outlier events. The odds of something like this happening on a flight you're on are still infinitesimally small. Consider how many flights take off and land each and every day without incident.

    It's happened more often than that. Remember the Egyptair 767 that the copilot crashed into the Atlantic? Or the missing Malaysian 777, which is believed by many to be a murder-suicide?

    At least most countries now require two crew in the cockpit at all times; both the Egyptair and Germanwings crashes involved the suicidal pilot locking his mate out of the cockpit. (I suppose that if one pilot has to go to the lavatory, they are required to have a flight attendant take his/her place in the meantime.)

    Virtually impossible to happen on US airlines, never one person in the cockpit. Rest/bathroom breaks there is always another pilot or flight attendant in the cockpit.
